Introduction
Clinical governance plays a pivotal role in ensuring the delivery of safe, effective, and high-quality healthcare services. At Digisafe, we understand the critical importance of patient safety and adherence to mandated clinical safety standards.
Our collaboration with the Yorkshire and Humber Shared Care Record programme exemplifies our commitment to enhancing clinical governance and prioritising patient safety.
Who We Worked With
In partnership with the Yorkshire and Humber Shared Care Record programme, Digisafe has collaborated with providers and consumers across various regions and care settings, to ensure the safe assurance of patient data, to address the challenge of fragmented patient information across healthcare settings.
Recognising the need for a structured framework that ensures compliance with clinical safety standards and promotes patient safety, the programme chose to work with Digisafe.
What We Did
Our involvement in the Shared Care Record programme has been comprehensive and empowering. We have:
- Collaborated closely with the programme’s project and product team from design to deployment, ensuring that patient safety considerations are integrated at every stage.
- Conducted thorough reviews of clinical incidents and provided recommendations for improvement, reinforcing our commitment to continuous enhancement of patient safety.
- Worked with other shared care record teams nationally to share best practices and drive standardisation efforts forward.
- Conducted comprehensive hazard assessments of design elements, ensuring alignment with national standards and minimising potential risks to patient safety.
- Developed clinical assurance models and frameworks to be used at the programme level, empowering the entire workforce to prioritise patient safety and adhere to clinical safety standards.
- Engaged with clinical safety officers and provided clinical safety expertise to ensure safe patient data transfer, integration, and interoperability.
- Ensured that clinical safety remains at the forefront of the programme, from development to go-live sign-off.
Highlight at Shared Care Record Summit 2023
Our achievements with the Yorkshire and Humber Shared Care Record programme, particularly our collaboration with the Interweave product team, were highlighted at the first Shared Care Record Summit.
At this event, we showcased our expertise in clinical safety and governance and shared our developments., we delivered a workshop with some regional partners from Leeds Teaching Hospital and Yorkshire Ambulance Service.
As a result, we now provide subject matter expert guidance not only in clinical safety but also in shared care record programme knowledge to other sites.
Future Collaborations and National Framework
Building on our success, we continue to collaborate with other Shared Care Record programmes across England and now into Wales. We aspire to work with other teams and NHS England to define a national framework for shared care records. By sharing our expertise and best practices, we aim to advance patient safety and improve healthcare delivery on a national scale with core assurances and models provided to others.
At Digisafe, we are committed to driving positive change in healthcare delivery by prioritising patient safety and adherence to clinical safety standards. Our collaboration with the Yorkshire and Humber Shared Care Record programme proves our dedication to enhancing clinical governance and improving patient outcomes. Together, we are shaping the future of clinical risk management and setting new standards for patient safety and quality of care.
